I see it as a sign that we have arrived somewhat. The Laser Center realizes a chiropractor can be valuable source of referral. Money speaks. I think we are becoming more mainstream and a force to be reckoned with. For most times we are the one of the doctors a patient sees for neck and back pain. So we went from most MD’s not willing to work with us to some MD’s actively seeking our referral. It’s all about the money. And money controls politics. If we want to be mainstream we have to play ball. Does that mean compromises? Most likely. For a straight chiropractor it might be sacreligious, but for a Mixer it might be a welcome change. I think how we feel about becoming mainstream and how to go about doing it would be a very personal matter. But I, for one, is all for it. Let them spend money marketing to us! More money and recognition coming to us as a profession more power we will have politically and more patients we can help ultimately.

I’m purchasing a chiropractor practice and so will likely have some questions for you all in the weeks ahead. I see that you’ve all been kind to new docs in other posts I’ve read!

I’ve been working at a chiropractor clinic and have recently come to an agreement with the current owner to purchase it. I have my own separate S-corporation that I’m using to purchase the assets & intangibles.

Since I am technically a different practice (different EIN than the practice I am purchasing), how should I handle the existing patients that have been coming to me already? Will I need to bill for a “new patient exam” (99201)?

On some of these people I’ve just done an exam less than a month ago so it will seem strange requiring another exam just because they are a “new patient” for what technically could be seen as a new practice.

I am guessing that it will vary based on whether they are cash, insurance (I’m in-network with most) or Medicare. Would I be ok simply billing a 98940 or a 98941 without an exam or would that raise red flags? I figure that I could possibly do that with cash patients, or else give a free exam to those that are maintenance or have had one recently. Medicare I guess would frown on billing a 98940 without an exam though.

What kind of verbage do you use on re-exam to show people the chiropractor changes, more specifically what do you do if you see the changes, through rom, sub station scans, etc but the person does not say they are feeling any changes?
Every once in a while we get someone who has a really low awareness of their body, we have a woman who just had her first re-exam, amazing changes, and she is a totally different person than when she first came in due to chiropractic, but she reports not feeling much change. When she came in she could hardly move without pain, now she moves freely, smiles more (she has a pretty negative attitude) plus all the objective changes but still complains that she isn’t feeling change. Even her husband reports how much change she has had in a short time. We do a quality of life questionnaire when they first come in and at re-exams. Its kind of mind-boggling.
I am sure we will lose her soon, I’m just wondering how others deal wtih this for this case and future one’s like it. It’s easy to accept when people quit care because of money but that’s not the issue and the progress has been really great.
